Gentle Pressure Relief: The Role of Laser Glaucoma Surgery
By Dr. Kerline Marcelin, M.D.
When patients learn they have glaucoma, their immediate thought is often about managing daily eye drops for the rest of their lives. While prescription drops have long been the traditional starting point for managing elevated intraocular pressure (IOP), keeping up with daily drops can be challenging. Between forgetting doses, dealing with redness or stinging, and the ongoing expense, many patients find daily eye drop regimens difficult to maintain.
Fortunately, eye care technology has advanced significantly. Today, laser glaucoma surgery offers an exceptionally gentle, safe, and effective way to lower eye pressure often reducing, delaying, or even eliminating the need for daily medications altogether.
As a fellowship-trained glaucoma specialist serving Cortlandt Manor and the greater Hudson Valley, I want to demystify laser glaucoma treatment so you can feel confident in your care options.
How Laser Surgery Lowers Eye Pressure
Glaucoma occurs when fluid inside the eye (aqueous humor) cannot drain efficiently through the eye’s natural drainage network, known as the trabecular meshwork. As fluid builds up, intraocular pressure rises, placing stress on the delicate optic nerve fibers at the back of the eye.
Laser surgery does not involve cutting into the eye or creating external wounds. Instead, it uses precise beams of light energy to improve the eye’s internal drainage system, allowing fluid to flow out smoothly and reducing pressure naturally.
Depending on the specific type of glaucoma you have, we utilize two primary laser procedures:
-
Selective Laser Trabeculoplasty (SLT)
SLT is the most common and versatile laser procedure for open-angle glaucoma and ocular hypertension.
Unlike older thermal laser techniques that caused scarring, SLT uses low-energy laser light that targets only specific pigment cells in the eye’s drainage meshwork. This gentle stimulation triggers a natural biochemical response that opens up microscopic drainage channels without damaging surrounding tissue.
- Repeatable & Safe: Because SLT leaves tissue intact, it can safely be repeated years later if needed.
- First-Line Option: Major clinical studies now support using SLT as an initial treatment right at diagnosis, rather than waiting until eye drops fail.
-
Laser Peripheral Iridotomy (LPI)
LPI is designed specifically for patients with narrow-angle or angle-closure glaucoma.
In narrow-angle glaucoma, the space between the iris (colored part of the eye) and the cornea is too tight, creating a risk that the drainage angle could block suddenly, a medical emergency known as an acute angle-closure attack.
During an LPI, the laser creates a microscopic, virtually invisible hole near the outer edge of the iris. This acts as a relief valve, allowing fluid to flow freely behind the iris and keeping the drainage channel open and protected.
What to Expect During an In-Office Laser Procedure
One of the greatest benefits of laser glaucoma surgery is that it is performed safely and comfortably right in our Cortlandt Manor office suite.
- Preparation: We begin by placing local numbing drops on your eye, along with drops to prepare the pupil. You will be awake and seated comfortably at a specialized microscope, similar to the one used during standard eye exams.
- The Procedure: A specialized viewing lens is placed on your eye to guide the laser. You will see brief flashes of bright light and may hear soft clicking sounds. The vast majority of patients feel no pain at all, with only a mild tapping sensation reported occasionally.
- Quick & Convenient: The laser treatment itself takes only 5 to 10 minutes per eye.
- Immediate Recovery: After checking your eye pressure shortly after the procedure to ensure it remains stable, you can head home. Most patients resume their normal daily activities—including driving and returning to work by the very next day.
Key Benefits of Laser Glaucoma Therapy
Laser therapy has become a cornerstone of modern glaucoma management because it provides distinct advantages:
- Freedom from Drop Fatigue: It helps relieve the burden of daily drop schedules, missed doses, and medication side effects like red eyes or dryness.
- Proven Effectiveness: Clinical studies show that SLT lowers eye pressure as effectively as a primary glaucoma medication.
- 24/7 Pressure Control: Unlike eye drops, whose effectiveness wanes as hours pass between doses, laser therapy works continuously to regulate pressure day and night.
- Tissue Preservation: Modern lasers leave delicate ocular structures intact, keeping all future medical and surgical options open.
Taking Control of Your Vision Health
Glaucoma is a lifelong condition, but with early intervention and advanced laser therapy, vision loss is highly preventable. Whether you are newly diagnosed, struggling with drop side effects, or looking to lower your eye pressure more consistently, laser glaucoma surgery may offer the perfect solution.
At Hudson Ophthalmology, we combine advanced diagnostic technology with compassionate, fellowship-trained care to ensure your eyes receive the exact support they need.
